RN Case Manager Hospice of Holland is seeking a Registered Nurse to assume responsibilities as a Case Manager. This is a full-time position requiring 32-40 hours per week. The Case Manager would have general responsibility for overseeing patient care and administering and coordinating the care plan for our patients. This includes proper documentation, adherence to safety standards and proper medication administration and protocols. Strong communication, critical thinking and assessment skills are required.

RN Clinical Liaison In this role the RN would be responsible for the initial medical evaluation and assessment of the patient, completing the admission and transfer process once the patient is deemed appropriate and developing, maintaining and enhancing clinical relationships with all referral sources. Strong presentation, assessment and communication skills are important.

RN- Weekend On Call We also have an opening for an RN to assume on call responsibilities for weekends. The successful candidate would be part of the on call team that rotates coverage thru the weekends providing nursing services for our patients.
